Start Your EntryNeighbourhood health is at the heart of the 10 Year Health Plan and the shift of care from hospital to community. The Neighbourhood Health Framework sets out how integrated neighbourhood teams, neighbourhood health centres and new provider models will deliver more proactive, joined up, place based care - with the first wave of implementation sites and neighbourhood health centres already under way. Making this a reality depends on digital: shared care records that let multidisciplinary teams work as one, population health data to identify and support high priority cohorts, remote monitoring and virtual wards that bring hospital level care into the home, and tools that connect the NHS with social care, local government and voluntary sector partners.
This award will recognise the NHS teams using digital and data to enable neighbourhood and community-based care and improve experience and outcomes for local people. Judges are looking for initiatives which show how digital and data are enabling new, holistic models of care across organisational and sector boundaries - health and non-health alike - and will be particularly interested in examples bringing together multiple partners, with staff and citizens fully engaged in project planning.
Eligibility
- All NHS organisations (including providers, partnerships, and systems), General Practice and primary care organisations, and their social care, local authority or VCSE partners.
- Evidence must relate to a project, ongoing or completed within the 2 years up until the award entry deadline.
